Customer Responsibilities:

(1) Offloading of the delivery truck on-site or scheduling a terminal pick-up.  A forklift, roll-back or similar heavy-duty equipment is required to unload these lifts. 

(2) Residential delivery is an additional $150 charge by the freight carrier.

(3) Lift Gate requests/reqmts are an additional $125 charge by the freight carrier
